Camp E: Pizza and Pasta Workshop (Ages 9–12)

Before your child participates in a Life Plus class, camp, workshop, afterschool program, private event, or other activity, please review the following information. Participation and Assumption of Risk I understand that my child will be participating in activities that may involve cooking equipment, kitchen tools, knives, appliances, hot surfaces, food preparation, arts and crafts materials, educational activities, games, and other related experiences. I understand that participation may involve risks including, but not limited to: • Cuts, burns, scrapes, and other minor injuries • Slips, trips, and falls • Allergic reactions or food sensitivities • Injuries resulting from the actions of other participants • Risks inherent in educational, recreational, culinary, and group activities I voluntarily allow my child to participate and assume the risks associated with participation. Allergies, Medical Conditions, and Special Needs I understand that I am responsible for informing Life Plus of any: • Food allergies • Dietary restrictions • Medical conditions • Medications • Behavioral considerations • Physical limitations • Other information that may affect my child's participation While Life Plus will make reasonable efforts to accommodate disclosed needs, I understand that Life Plus cannot guarantee an allergen-free environment or eliminate all risks. Medical Authorization In the event of an emergency, I authorize Life Plus staff to obtain emergency medical care for my child if I cannot be reached immediately. I understand that I am responsible for any costs associated with such treatment. Release of Liability To the fullest extent permitted by law, I release and hold harmless Life Plus, its owners, employees, instructors, contractors, volunteers, affiliates, and agents from claims arising from my child's participation, except where prohibited by law or caused by gross negligence or willful misconduct. Photography and Media Life Plus may photograph or record activities for educational, promotional, website, social media, or marketing purposes. By allowing my child to participate, I grant Life Plus permission to use photographs, video recordings, or audio recordings that may include my child's likeness. Program Conduct and Safety I understand that Life Plus strives to provide a safe, positive, and respectful environment. I understand that Life Plus may remove a participant from an activity if their behavior creates a safety concern for themselves, staff, or other participants. Parent/Guardian Responsibility I understand that I am responsible for complying with any program-specific arrival, dismissal, pickup, and attendance procedures communicated by Life Plus. Electronic Acceptance By selecting "I Agree" and registering my child for a Life Plus activity, I acknowledge that: • I am the parent or legal guardian of the participant. • I have read and understood this waiver and acknowledgment. • I voluntarily agree to its terms on behalf of my child. • My electronic acceptance is legally equivalent to a handwritten signature. • I have the legal authority to enter into this agreement on behalf of the minor participant. • I am the parent or legal guardian of the participant and agree to the Life Plus Youth Participant Waiver & Parent/Guardian Acknowledgment.
