Before participating in a Life Plus class, event, workshop, private experience, or other activity, please review the following information. Participation and Assumption of Risk I understand that participation in Life Plus activities may involve the use of knives, cooking equipment, kitchen appliances, hot surfaces, hot liquids, food ingredients, and other materials commonly found in culinary and educational environments. I understand that participation may involve risks including, but not limited to, cuts, burns, slips, falls, allergic reactions, food-related illness, property damage, and other injuries that may occur during or as a result of participation. I voluntarily choose to participate and assume all risks associated with my participation. Allergies and Dietary Restrictions I understand that Life Plus operates in a shared kitchen environment where common allergens may be present. While Life Plus will make reasonable efforts to accommodate disclosed allergies and dietary restrictions, Life Plus cannot guarantee that any food or environment will be completely free of allergens or cross-contact. I am responsible for communicating any allergies, dietary restrictions, or medical concerns prior to participation. Release of Liability To the fullest extent permitted by law, I release and hold harmless Life Plus, its owners, employees, instructors, contractors, volunteers, affiliates, and agents from claims arising from my participation, except where prohibited by law or caused by gross negligence or willful misconduct. Photography and Media Life Plus may photograph or record activities for educational, promotional, or marketing purposes. By participating, I grant Life Plus permission to use photographs, video, or audio recordings that may include my likeness. Alcohol If alcoholic beverages are served during an activity, I certify that I am at least 21 years of age and will consume alcohol responsibly. I understand that I am solely responsible for any decisions related to alcohol consumption and transportation. Participant Conduct I agree to follow instructor directions, use equipment responsibly, and conduct myself in a manner that promotes a safe and enjoyable experience for all participants. Electronic Acceptance By selecting "I Agree" and participating in a Life Plus activity, I acknowledge that: • I have read and understood this waiver and acknowledgment. • I voluntarily agree to its terms. • My electronic acceptance is legally equivalent to a handwritten signature. • I am at least 18 years of age and legally able to enter into this agreement. • I have read and agree to the Life Plus Participant Waiver & Acknowledgment.
